Allscripts Professional EHR is a solution for small to midsize physician practices. Professional works in accountable care organizations (ACO), patient-centered medical homes and federally-qualified health centers.

Professional EHR tracks the history of what care providers have given in previous treatments and can repeat when applicable. Users will receive patient care suggestions based on patient specifics like age and symptoms. Caregivers can then filter their lists for diagnosis and plans based on their individual preference. SIGs for smart list medications will also be auto-populated based on patient specifics like age. Professional EHR can sort views on the face sheet based on user preference.

These views include a duplicate diagnosis view that groups identical diagnosis codes in one view, a problem category view that groups diagnosis codes into like categories and a specialty-specific problem view that groups diagnosis codes into categories relating to specialties identified in caregiver options. Cons Lots of clicks to do anything.

Pro

Requires several screens to do almost anything, with slow transition time between screens. Used this 68 months, seemed slower the longer we used it. Certain parts could not be edited. Requested changes re: typos & inaccurate descriptions which could not be edited, and still were not fixed after years and multiple Updates.

Frequent freezes and crashes. Anytime an Update was implemented, several days of freezes and crashes.

Cloud

Many Updates worthless (e.g., changes in screen colors, fonts, design of screens, etc.). Template outputs too wordy. Not intuitive at all, needs 12 clicks to do pretty much anything. Progress notes are not searchable for data(no snomed assignment of findings) very little is, so documenting quality parameters (e.g. Diabetic foot exam or depression screening) means going outside of the progress note (12 clicks) and separately entering an order to do the exam then satisfy the order with a result. They kind of cobbled their data processor to a word processor. The front office scheduling and billing module is often down, and Allscripts is very slow to respond and not proactive, only slowly reactive to problems.

Updates to the EHR tend to screw things up (like deleting favorites lists) and because of their slow responses, it takes 3-4 weeks to regain/repair that lost functionality. Our previous EHR would do beta testing on updates first which would identify glitches so they could be repaired before full rollout and we would be given the option of being a beta test site or not. Templates for progress notes are primitive and not interconnected. For example, a diabetic visit and patient has sinusitis too. Clicking finding on one form doesn’t show same item already clicked when using any other form.

And the History forms are not linked to PE forms, etc, so if you have a Lyme patient you can search for a Lyme history form (12 clicks) but you would separately have to search for a Lyme PE form (12 clicks) or just give up and free text hx and PE findings (thus functioning more often as a glorified word processor). Cons Constant 'upgrades' that didn't do anything to improve productivity, only made it more difficult to complete my daily work. Customer support was terrible. Biggest drawback, now in hindsight, is how incredibly expensive it is. We started with them early when there weren't many options.

I'd keep hearing from other docs how much they were paying for their EMR's and be thinking 'no way, they must not have what we've got'. Turns out, I was the one being had. I now spend less than half of what Allscripts charged me, and get office management and billing support as well. I have trouble understanding how they are still in business with their model. Pros You can take breaks between patients while the EMR goes down quite frequently.

The patients don't like it though when they start a riot in your waiting room. Because the software and cloud are terribly made and break down a lot you do get to talk to the folks in India a lot.that's where the IT is based from.

Since they aren't able to help you and just say things like 'this is a global problem we are aware. ' That's really nifty but I have 15 patients waiting for me. This hasn't really turned out to be a PRO.
